Layered Loop Philosophy

Building on the Systems-Theoretic Process Analysis (STPA) research performed by Nancy Leveson from MIT and numerous other researchers, our team has developed a layered loop diagram. This diagram demonstrates the relationship between the flow of “direction and context” from sources like the regulatory body, public opinion, and Corporate Sr. Management, its direct and indirect impact on plant operations, including the effects of erroneous and imprecise direction, and how the feedback from the various lower control loops (including errors, filtering, and dissonance) ultimately affects the future direction and context.

Marvin J. Dainoff, PhD, CPE
Dr. Dainoff has a rich psychology and academic research background, Dr. Dainoff initially served as a professor at Miami University between 1969 and 2005. During this time, he and his wife, Marilyn, founded Marvin Dainoff Associates, Inc. Consultants in Workplace Ergonomics, and he served as its president between 1982 and 2008. He was also a former director at the Center of Behavioral Science, Liberty Mutual Research Institute for Safety.
Lawrence J. Hettinger, PhD, LLC
Dr. Hettinger specializes in Human-Systems Integration, Complex Systems Safety and Performance and is a former Principal Research Scientist at the Center for Behavioral Sciences Liberty Mutual Research Institute. Dr. Yamani is a tenured associate professor and associate dean in the Department of Psychology and the Department of Civil and Environmental Engineering at Old Dominion University, Norfolk, VA. John M. Flach, PhD
Dr. Flach established Perspicacity, LLC in 2023 to provide consulting services to explore and improve performance in sociotechnical organizations. John has extensive experience in the fields of Applied Cognitive Psychology, Cognitive Systems Engineering, and Ecological Interface Design, where he has contributed to the development of both theory and practice.

Jeffrey C. Joe is a distinguished Research Scientist in Idaho National Laboratory’s Human Factors and Reliability Department. Robert Fisher
Robert Fisher is a distinguished leader in the Nuclear Industry who is a native of Cleveland, Ohio and a graduate of both the US Naval Academy, 81' and Penn State 87'. He has served in the US nuclear navy and has lead numerous nuclear utilities in the US and around the world. He is the author of a number of personal and professional publications.
